In addition, NCQA has announced that a new 2014 version of the Patient-Centered Medical Home (PCMH) program will launch on March 24, 2014. The new program is designed to align more closely with Meaningful Use Stage 2, emphasize team-based care, align quality improvement activities with the “triple aim,” and put a care management focus on high need populations.
